We are contracted providers for Medicare, Medicaid (Public Aid), Access DuPage and most insurance plans.  After your examination, we will submit a claim to your insurance carrier. 

 

You may be responsible for a "co-payment" at the time of your visit.  Your insurance card will usually indicate the amount of "co-payment" usually between $10 -$40, depending on your plan.  You may also be responsible for any unpaid deductible amounts on your insurance.  Most insurance companies do not pay for the cost of a refraction.  A refraction is the testing done to determine your glasses prescription.  Our charge for this service is $32.    

 

If your insurance plan requires, please secure a referral and bring it with you to your office visit.  If you are part of an HMO, POS or EPO you are probably required to have a referral from your primary doctor in order to see any specialist.  If you do not have your referral we will have to reschedule your appointment.  We have to abide by the rules of your insurance carrier.  If you have questions regarding your insurance benefits, eligibility or referral requirements please contact your insurance company.
